Medical Financing and Medical Benefits
IMPORTANT: AmeriPlan Health is NOT insurance.
- A membership for AmeriPlan Health/DVPC costs:
- $49.95 - per month for an individual
- $59.95 - per month for an entire household
- $30.00 - one-time registration fee
- Florida Residents Only
- $19.95 - Per month entire household DVPC
$20.00 - One time registration fee
- $30.00 - Per month entire household Medical
$30.00 - One time registration fee
- $49.95 - Per month entire household DVPC/Medical
$30.00 - One time registration fee
- Membership activation process may take up to 10 days or more if the application is incomplete in any way.
- Approved individual member receives a Member Information Guide and two ID cards - one for AmeriPlan
Health® and one for DVPC (same as present card).
- Approved household member receives a Member Information Guide and four ID cards - two for
AmeriPlan Health® and two for DVPC (same as present card).
- If member requires medical attention prior to receiving their AmeriPlan Health® ID card, they may call
AmeriPlan Health® Customer Service for assistance in accessing medical services.
- Medical Provider Network is composed of:
- Primary Care Physician (PCP) - These are general practitioners such as your family doctor.
- Physician Specialist - These are physicians that have an advanced degree in a specific area
of medicine such as OBGYN, Cardiologists, etc.
- Ancillary Medical Services - This includes other medical services outside the physician’s
office such as lab work, imaging & radiology and hearing services.
Hospital Confinement - The AmeriPlan® member will be guided through the hospital process by our contracted patient
advocacy program.
